DESCRIPTION
Social determinants of health (SDOH) are factors that influence one’s level of health literacy. Low health literacy is often a contributing factor to difficulty managing chronic cerebrovascular conditions such as diabetes, hypertension and hypercholesterolemia. Poor management of these conditions may result in vascular cognitive impairment. Healthcare professionals have a unique role in improving the health literacy skills of our patients with vascular cognitive impairment. This webinar will demonstrate how through incorporating elements of health literacy and interdisciplinary collaboration into their treatment plans, healthcare professionals can improve holistic knowledge of the body for their patients. This webinar will further explore how SDOH Neighborhood & Built Environment has contributed to health inequities that lead to vascular cognitive impairment. Learners will leave with strategies to incorporate health literacy into functional goals for their patients.
LEARNER OUTCOMES
Upon completion of this webinar, participants will be able to:
- Define health literacy, vascular cognitive impairment and identify three social determinants of health.
- Identify risk factors for vascular cognitive impairment during a chart review.
- Identify three holistic therapy ideas to incorporate overall health and well-being into healthcare services.
SUCCESSFUL COMPLETION
